I have an August lock. When locked, the state updates almost immediately in home assistant. However, in the app it appears to only update when I minimize and reopen.
Does it happen after lock action from within the app or through HA UI?

Ok, there is currently a bug that makes it go out of state for some devices. Usually depends on how fast device is responding on action, so it shows for some devices more than others.
The fix is in the works.
